If I had to guess, that looks like quarters that have been shredded by coin wrapping machines and gumball machines, and then smoothed by heavy use. I think slot machine coin is a good suggestion.

Interesting coin. Beat up for sure. I can't say if it's PMD or a Greaser.

Definitely not all slot machine wear. There is weakness in certain areas. The weak reading can happen for more than one reason.

The rim is raised because the reeds have gotten beat in - the metal has to go somewhere..
