Is this an error or PMD? If an error, is it worth keeping?

Doesn't look like anything PMD related to me. If so wouldn't you still see the outline of the original "S?" I don't know what caused the error but I would say that it's an error of somesort.

In the same circular plane as a coin wrapper machine but suspect some other type of mechanism, coin counter?

It's always tougher to accept date and mint mark hits as nothing more than PMD, but that's what it looks like.
