1982 D Washington Quarter Errors?

Aloha!

I need help with my Washington quarter- On the obverse, the left side it looks like an embossed letter or number on the collar and on the right side it looks like a hole punch on the collar.

On the reverse side it looks like the whole quarter around has disappeared, including the tops of "united states of America" and the bottoms of "quarter dollar".

On the outer collar all around the quarter is all smooth...Any feedback would be greatly appreciated.

@gsi, while that does look quite a bit like a number 5 on the rim, I think that it actually is a rim ding plus Pareidolia.
