I can't offer any input on the variety, but grade looks to be in the low MS range. That obverse is REALLY baggy.

MS-60. Looks like a die scratch and not a doubled mint mark.

I took some more photos, this is at 10x magnification, at different angles and lighting, looks like it is doubled or over O O to my eyes, but I can't find any other examples in this year so I am skeptical about that, could just be a mint mark that got severely beat on by other coins in a bag?
