The bow on the ribbon seems to be completely disconnected from the ribbon. Is this a common error? Or not an error at all? Thoughts?

Several possibilities:
-- later die state
-- die polishing
-- weakish strike

Good images of the full coin, both sides, might clarify that.
Describe it as if there were no picture.
Picture it as if there were no description.

I would lean towards fake. Just don't look right

After comparisons with genuine examples, I agree this is a counterfeit.
