I would guess a die dent. The way dies were made back then makes it unlikely that you would have just the center die doubling effect that we see on modern coins without any other doubled areas. When they used to perform multiple hubbings, doubled dies would happen when the hub was applied out of alignment with the previous one between annealings. These center die doubled dies happen on single squeeze presses when the hub is applied slightly out of alignment with the die and it snaps back into place during the hubbing process.
