1978 Quarter Reverse, A In America Missing

I'm a noob so please go easy on me. The America on the reverse is all but missing. I was wondering if it is grease,a Cud or a dude with a file. On the rim of the coin is a crack where the A would be. Also it appears as if there is some faint lettering beside the other A in America. I would really appreciate your opinions. Sorry the pictures aren't the greatest. Thanks.
