What would be a fair price for a coin like this? It is an an old rattler holder and appears to be a high-end 65. I'm trying to decide what my best offer should be. Numismedia FMV is $225.00.

Unless it's a toner or otherwise really special, I'd offer $150.

The last 6 similar results from Heritage, all PCGS 65's, all this year: $138, 322, 161, 299 (toner), 161, 161. I have no explanation for the $322 outlier.

With that said.... It depends on the quality of the strike. In my opinion, Weak struck coins are passer "byes". Well struck examples might go as high as FMV or higher depending on the date and mm.
