Host coin is a Giorgino from the Italian state of Modena.

No idea on the counterstamp.

Someone told me the counter stamp is from St Martin island.
I was more curious about the host coin.

What does it mean for this coin? Is the counter stamp
a fake?

It means probably that the counterstamp was illegally placed by a non-royal mint. As the host coin is Italian, this could have been done in Avignon (papal city) or Orange (capital of the principality of Orange).
