The people depicted are Queen Elizabeth II of Britain and her husband Prince Philip, Duke of Edinburgh. But the language in the legend around the portraits is Italian.

It isn't a coin, or an official government-issued medal. I assume it's a privately-made medal. But I'm not sure why Italians would want to make or buy a medal with foreign royalty on it. Perhaps it was made for sale during an official visit to Italy.

I don't know for sure how old it is, but I can speculate: The couple do not look as elderly as they do now, so I assume it was made sometime around the 1960s-1980s. According to Wikipedia, the queen has made only two official state visits to Italy, in 1961 and in 1980, and I'm not entirely sure the Duke accompanied her in 1980. So my best guess is that it dates form the 1961 tour.

Sap, you may be right, but that bust looks older than 60s Lizzy.

I reckon that 1980 sounds just about right. Look at some UK coinage and make note of the bust from 1983 onwards, when it was updated.
