Most likely discolored due to environmental or chemical exposure. I have found dimes that looked completely copper that had been lying in dirt for some time. The minerals in the ground react with the surface of the coin to change it's color. If your nickel weighs the same as another normal one, then that is what you have.

Is there any real way to know if a coin that has lost plating is not the result of acid or PMD. if there was a way to prove it had lost its plating as a result of a minting condition, would the coin be worth much more than face?

The only U.S. coin that is so called plated, is the copper over zinc. one cent. It is possible that some unplated cents
can be minted. There are ways to tell if so and that they are not PMD. This nickel looks strange. It's a nice strike and in good shape. No idea what caused it. Possible alloy problem ?
Maybe. maybe not.
