Here's what looks like to be a fairly crudely made medal. First thing that struck me is that it is out of proportion in many areas. Appears it made have been made by hand and I'm not sure of when it was made.

On the obverse side check out the guys leg through the spoke (one turn and ouch ). Horses tail looks more like that of a beaver and horses hind legs are longer than the front legs. Under the horses belly the semi-circle from the reverse side struck through as if hand hammered too hard. Not sure what this represents...

On the reverse there are two houses, a semi-circle (sun ? ) a bird and the rest I don't know....

Any ideas...?

The cart looks typical of the type used in Japan (perhaps China). Like wise the building is from the same region( I am leaning more to Japan). Yet this medal type seems reminiscent of 18th century Europe. As a guess I would venture the possibility it is an issue produced for a trading mission to the far east(as a gift/presentation).

I agree with Japan. Other than that, I can't say much about it.

D, I'm wondering if this isn't a lot later. The original disc has machine made denticles similar to some gaming machine tokens or, considering the apparently machine made hole, tool checks.
The V shaped chevrons on the reverse seem more at home in western art than Japanese, and I'm wondering if this was maybe a form of Hobo artwork by a post WWII American GI.

Bob..great suggestion, I generally think of hobo art as on nickels but I think you have a very good point as this little (22mm) bronze medal is all done by hand...I'll have to do more research in that area....
