I bought a PR69DCAM 1981-S type 2 dollar that looks to have a significant spot, scratch or something on the back. Do you think this would be covered by their guarantee?

That's why they certified ms69 and not ms 70

And there is a good chance it isn't covered anyway. From the PCGS guarantee under things not covered (emphasis mine)

Coins exhibiting environmental deterioration. The PCGS Guarantee does not apply to coins exhibiting environmental deterioration subsequent to PCGS grading and encapsulation. This deterioration may include, but is not limited to, spotting, hazing, PVC contamination, changes in color, and corrosion.
