I should mention that this mis-struck shilling is in an NGC slab. (Guess I should have scanned the holder too).

How on earth did such a coin ever end up leaving the mint? You would think someone would have spotted it and thrown it away.

Could you post a picture of that from the side? Show the rim and the obverse in one shot if you can please.
