Quite a few of us are using old duplication lenses for coin photography, and these tend to be M39/Leica mount. Is there a fairly standard size for the filter threads on the popular ones? My Apo-Rodagon-D is 40.5mm; is that more-or-less the norm or is it a pretty wide variety?

40.5mm is the most common. I'd say perhaps 1/3 of the lenses I have use it. Early Nikon was 34.5mm. Beyond this, there is a hodgepodge of different threadings, though most are standards like M43, M49 or M52.

With your "smile director" lighting modifier you needn't be too worried about filter sizes. Its fairly easy to get step up step down rings. For example 37mm-40.5mm:
