I am new to this forum and can use your help. I inherited a coin collection from my grandfather, and found this item in the collection. I haven't been able to find anything like it online so far. I don't even know if it is a coin. The back of the item is flat with a bunch of random grooves and scratches. Does anyone have any ideas? Thanks in advance!

My first impression was that it is some sort of modern 'imitation' of a Roman republican coin.
Regardless how the reverse looks, show it and tell us also the diameter of the object. Perhaps one can make out which specific type is imitated here.

This tells me the piece was likely intended to be mounted into something large, with only the portrait able to be seen. Perhaps the cover of a book, or an old piece of furniture.

Thank you for your replies. I apologize for the belated response to the questions posed. The attached photos are of the obverse, reverse, and rim of this object. It is approximately 1-3/8 inch in diameter and 1/8 inch thick. It weighs 0.9 oz.

I hope this helps someone identify what this item is. So far, my favorite theory is that it was a medallion affixed to doomed old bottle of wine or champagne! Any ideas or suggestions are greatly appreciated!

The filed edges suggest a jewelry mounting of some sort...I suspect for a medallion. The image depicts a helmeted Roma based - in a generalized way, as Medieval suggests, on a Roman Republican denarius. Here's an example of the general type it borrows from:
