are you sure it is on the coin? if not you can put the ICCS flip on a table on its side and tap it a couple times lightly so the coin moves a little. if in the same spot its on the coin, but if it isn't on the same spot than it is a minor bend in the flip which is very common for these holders.

T1Steel...Looks like the "hole" is deeper than the metal between the beads..As the picture below shows, the "blue" arrows are pointing to the area between the beads at a certain depth..The "yellow" arrow points the the "hole"..Darker the colour the deeper the hole..Something gouged into it, made a hole and pushed the metal up beside it..Not sure what made the hole though..?
