Most of the block lists are crowdsourced - meaning SOMEBODY reported the site. They also often exchange information, so it can propagate from one list to another.

All of the reputable vendors have a report incorrect entries mechanism and they usually do a quick check and unblock it - sometimes it takes a day, frequently just and hour or two.
