I found this coin in a vending machine today, I've yet to find an error coin so I was wondering if you could tell me how this happened? Thanks!

It's in the same location on the obverse and front. Is this a die error?

The coin was stabbed with something on the reverse which caused the bulge on the obverse, PMD.
