Not sure if this is MAD or something to do with the die being at an angle. Maybe just PMD. Any input appreciated!

You have it right: MAD obverse. LDS 1989 with a blemish on the date, but a nice coin to find in change or roll hunting. You can see the heavy polishing around the outer devices. The letters look sunken.
