This is some PMD (post mint damage).
A doubled ear would be an exact replica of the ear/ear lobe and be mostly located next to the original. If the die were to turn between strikes, it would double the entire image in whatever way it turned.

The thing is that the lines going down his jaw are all single lines in the exact shape of the outer ear. The large chunk that looks like a rotated ear is coming outward, not sunk in like I assume would happen if someone hit it with anything. I guess maybe a better camera would do the trick.
