I found this in a BU OBW 1995 P LC roll a long time ago. It's a very interesting struck through error and it's in a unusual place It's on the 1st Memorial Column.

It looks like a "spiked head" or "cracked skull" as well.

Also, I suggest removing the coin from the flip for pictures if possible. The plastic really distorts/mutes the coin's surfaces.
