i found this Jefferson nickel 2015d and I see an extra eye ... what the experts see ?

Looks like PMD to me. Just very coincidental to look similar to the eye design.

I can certainly understand why you would ask.

Dings that somewhat resembles a third eye, but it's not a mint error.

I'd still keep it as an example of weird bag marks.
