The die chip is an individual raised line - not tripled.
On closer inspection the date is into the inner rim and after checking a normal coin I notice the stalk end is closer to the inner rim. Slightly off centre strike causing the date to pierce the inner rim.
Numis report triple rim strike errors, but cant find an example to compare.
