Are the lines across the coin from the die being polished?

Steve199, yes they are polishing lines, note the pebbled look, that is late stage die flow movement. A good strike and unusual in that this is a reverse MAD, usually the reverse die isn't out of alignment that much, if at all, and the hammer die holder is what gets loose!

What keeps this from a 66+ or 67? MAD?

Not really the MAD in my opinion that keeps it from 67. There are some very small nicks on the obverse and reverse. In grades above 65, small nicks like this are very important. It has excellent eye appeal for a 66.
