OK in most places, but looks to have some features of casting in the eye and ear area on the obverse, and close in to the edges of the design near shoulders and torso, on the reverse.

What is the weight and approximate diameter?

The Lion headdress is not as defined as what I would expect to see based on what the rest of the obverse looks like. And eye looks very odd.

Here is how I think of things like this: Ancient coins that are real have hints of all features of the strike left there. Always. Look at how sharp the dotted border is on the obverse. That indicates, as Echizento alludes to, a coin that will show all it's features to some degree as the years have not been rough on the coin. Not look at the lionskin headdress: That is not just smoothed with age features, those seem to be features missing that should be there. So much more detail would be expected on that headdress and it appears it was never struck into the coin. Then compare others like below. Now you hope to see an example where this is the case when you start to worry about a coin. And sadly you come up with an obverse you can't trust. Sorry. And even a smooth worn tetradrachm of Alexander will show some features but let's assume the smoothing of the lionskin was complete. Then the dotted border would be smooth as well and it is not. Sorry:
