I am having a problem understanding how to distinguish between the FH, fullhead, and normal head for all three types of SLQs.

Are they the same guidelines for all three types: 1, 2a and 2b?

That's a pretty fine distinction - I'm sure we've all seen worse than the top one in FH holders.
