wondering what would cause the issue on the reverse side, also leaving the small circles as to never being touched

There is not a damaged die, but it is considered environmentally damaged coin due to staining caused by unknown liquid substance.

just some type of environmental damage on the coin - coffee, pop, chemicals etc....

Just a stain. It's not an error, just Post Strike Damage ( PSD), meaning that this happened after it left the U.S. Mint. This cannot happen at the U.S. Mint. It's impossible.
