The "hanging" part is a die clash from the line of the queen's chin/throat. The raised dots are from ?ghosts? It's not rotated. The coin is not placed in the 2x2 correctly. The I of DEI is not the topmost point. The bottom centre of the coin is halfway between REGINA and ELIZABETH. The queen's face is supposed to be at a slight upward angle.
