I have just found a 'standing on the shoulders of giants' £2 coin with a capital letter Q stamped neatly on both sides.
The reverse side is right in the middle of the coin and the other side is just off centre.

Question: Is this strike made during production or is someone defacing coins with a Q punch

your thought would be appreciated

Yes its just been defaced with a Q punch.
If there had been any reason to do it at the mint it would of been recorded and not put in circulation IMO.

It's called a counterstamp.
Someone for some reason has punched it after it left the mint.
It adds no value to the coin.
