Seems to be a $10 gold 1913 Canada coin, with a strike from a 2004 Canada CNA president medal.

Any idea on the story and the rarity of this. Do you think its legit or faked?

Probably a legit gold coin that has now
been damaged. Still worth bullion value.

Yeah, I was thinking around $900 Canadian BV. Since traditional the $10 Canada hoard coins are 90% gold 10% cooper. As for the error, it is probably an after mint error, since the 2004 medal was not mintded at a RCM?

It's the same crappy idea as the JOP dollars and the recent overstrike of the 67 dollars that some novice collectors are paying 300 bucks or more for a 15 buck coin

There are a number of different Chuck Moore overstrike medals out there, most are unique. There is a small market for those... I more or less see these as exonumia. Here is another example.
