2002-P Roosevelt Dime - Interesting Extra Metal In Eye And On Reverse

The torch looks to be a die chip but the left olive branch has raised metal along the stem and on / between leaves.

Roosevelt' eye is the really weird looking issue, the spike coming out of the eye and extra metal in the socket and below.
