You can see there's an indent starting from the E and ending at the R, it doesn't seem man made to me, there isn't any damage on the reverse side or rim on both sides. The R is very slanted and possibly showing a little shadowing. opinions y'all's?

Looks like a stain to me and circulation wear and flattening, from taking hits, on the R. PSD is my vote.

There's a fairly good size indent in between E and R, but that can be caused by normal wear and tear? Seems like an odd spot especially since the coins let's everywhere else is clear and in good condition

Ayotech1, yep. It's in a odd location, but it happens. Your coin has seen a lot of circulation, since it's 23 years old. Used to be in MS BU condition at one time, when it left the U.S. Mint. Now it's very worn down. It's unfortunately not an error coin.
