/1/ The term is doubled die - as in the die itself is doubled which is transferred to the coin.
/2/ No, it's machine or Mechanical Doubling (MD) - notice the shelf on the T in your first pic? That's the sign. A true doubled die would have two overlapping impressions and be either thicker or more telling have slight splits where the overlap isn't perfect. The shelf is because with MD, it's the planchet that shifts during the strike.
