2006 Colorado ATB Quarter REV And Obv. Damage? Or Errors? Not Really Sure...

Looks like there are holes on top, bends and folds on edge, and the thing that caught my attention most is the obverse just to the right of the word "IN" GOD WE TRUST. A penny for your thoughts...
I circled this area because the pic is not really showing that this mark looks similar to the devices on the coin, such as the date and mint mark. If that makes sense..

Also, on the reverse, I see something above the bottom date and another below the top date.
