2006 P Nevada State Quarter: Need Help Identifying Possible Error

So this could be a combination of problems, or simply some bizarre PMD. I'm no error expert.
-Core is showing through below the center of 'OF AMERICA.' -Bottom of the cheek/jawline is lower than the rest of the face, texture of metal is odd which my first thought was lamination loss post strike.
-Two incuse regions directly in front of the nose. Pre-die crack depression? Remnants of a clash?
-Now onto the obvious parts; two places in the recessed areas of the face & neck. In my opinion they are too smooth for road rash. The larger one of the jawline almost looks like a meteor. Reminds me of some of the surface fissures on 45' Lincoln Cents from the strange mixtures of metal used.

Tried playing around with possible clashes and post mint damage scenarios. These are the two that seem to line up with the ridge on the cheek

Can post more photos at pretty much any angle/lighting setup and can crop these in if needed.
