Looks like an English silver medieval groat, almost certainly genuine. It's been clipped at some time and some of the legends are missing. I'm sure someone will chip in with a more positive identification soon. It's a London Mint issue (Civitas London) on the reverse. These coins were minted for over 100 years with only minor changes to the design through several reigns.
