I came across this in a recent recently purchased box of coins, I would like a few opinions on whether its an error or something else PM ? Sorry the first picture is terrible ? Shaky hand ha

Not an error it is a lamination problem.
Put it in a 2x2 to protect that loose piece from falling off.

Looks like a lamination to me as well, but sharper pics would clarify.
