1968 D Lincoln Cent Memorial Column Error

Came across this during a recent roll hunt. Obverse has some what I think is de-lamination on the rim and IN GOD. And the 1st Column on the reverse has an issue - Don't know if this is PMD, die chip, DDO, die crack, interior die break? Any help would be appreciated.

Obverse rim looks like the result of circulation wear and contact damage. Reverse looks like a small die crack/die chip.

How many coins are typically struck to before an error like this is discovered and remedied?

That is a great question. I guess it would depend on the operator in charge of he machine at the time.
