I'll take a stab at it...looks like a major mint error to me. Perhaps a damaged planchet. The Indian looks like it was minted over the damaged planchet and not the other way around. You can still see the Indians lip, chin and neck. Let's see what others think.

Not an error. Looks to be a 'vise job' where someone hammered or squeezed another IHC into this one. It's just damaged.

But if you look, the punches are not backwards. They are straight on spelling. JW

The second punch is backwards.

The UNITED is forwards because that's from the 1904-dated coin. But the second punch caused by damage from a second coin shows "UNITED STA" backwards.
