I have a 2001 "P" nickel that seems to have only the obverse side struck with the reverse side almost completely blank. I am by no means an expert in coin collecting or coin identifying. Here are a few measurements I made with my digital caliper to help figure out if this coin has a true minting mistake or if it looks this way due to use. I will also attach some photos as well. Any help would be greatly appreciated and don't have high hopes that this is a true minting error.

Diameter: 21.32mm
Thickness: 1.65mm

Your coin is PMD not an error.

What happened to your nickel is very unfortunate. Somebody decided to deface it with a sander or grinder and remove the surface of the coin.

looks out of round, if so, it may have been flattened by hammering
