The picture isn't very good, but looks like it just took a slight ding in circulation and some metal got displaced. Not an error.

It might be PMD from a hit. Picture is too blurry to be sure.

Might be BIE B105, 107 or 109. They are all the same coin, just different stages. Does it have die cracks here and there? Would be good to show thos if they are present since they are diagnostic for the BIE.
