Filled MM are not unusual, the post on the die just broke off.
This coin looks to be very heavily circulated. PMD

There seems to be a faint Ring of Death around the outside of the quarter.

I don't know much about coin errors or how they happen, I can only recognize the most common ones. But to me this coin looks normal as others coins for the age, I am penny and quarter roll hunter and I saw a lot of coins like this, in fact I can say that all the 1982 and 1983
i have seen in bad or good condition look like that on in God we trust. And the mint mark, the IN are melted almost all of the time (don't know if melted is the correct word, sorry for my english)

Im still learning.

Your coin is PMD. Circulation damage/flattening. Also looks like vending machine damage (ring scrape).
