Can someone help me determine what this type of error is called ?
Is it a known error?
Does it add value to the coin ?

The "6" or "9" is located on the reverse side in the letter "A" in States
1921-Morgan-Dollar-Philadelphia-Minted-With-A-

Yes, I see the flare area where the number sits .

Am not sure what a circulation hit means . Can you explain or drop me a link ? ( I just tried to find online but was unsuccessful.

Am trying to understand if the 6 or 9 came from a strike from another coin
And also understand if this is rare enough to add value to the coin ?

Circulation hit means it was damaged while in circulation, hit with something, also called post mint damage PMD, it did not come from the mint this way, not an error. It's probably just a coincidence it looks like a number. Or someone used a number punch to make the impression. People do all sorts of things to coins, heat them, try to make a false toned color, scratch letters on them, stamp them with designs, put them in a vise with another coin to force a design to the surface, make a fake error, no value in any of this.

To be a true error coin it has to have happened at the mint, the area on your coin did not happen at the mint, no extra value.
