What Is This? No Obverse Jefferson Nickel

Can anyone tell me anything about this coin? I'm fairly certain it must be an error, it does not look like damage, and the thickness is the same as any other Jefferson nickel. I do not know the date. The obverse is completely smooth, with a small Crescent fingernail shaped indentation, and an irregular shaped crater. In the middle of the obverse, there's a very slightly raised circular area.

I'll say PMD sanded, ground down after it left the mint.

@TX_CoinLady - Sanded or ground down. Not an error, just deliberate damage. . Are those three pictures of the same coin?
