PCGS - No, this coin doesn't have crazy toning. It was just struck on the wrong planchet. This Jefferson nickel has a copper color and broken details after being struck on a bronze cent planchet. In PCGS MS62RB, this piece is a charmer.

Yes, much more plausible. I'm sure they don't have designated containers for each type of planchet to move them around. A few could easily be left in containers here and there for the next haul.
