I am continuing to add information as I find additional Certificates and plan to post updates in the future. I have posted this information in several venues and find some folks stepping forward (as some have in this thread!) and volunteering copies of their certificates for addition.

I also have copies of almost every certificate listed in the spreadsheet, along with a few photos of dies in those cases where there is an unusual feature - especially any evidence of the damage (which is rare).

I assume the Olympic dies were withdrawn from service soon, not many strikes because quality control was high for them. I suppose the dies may have had a problem but I doubt that. It's interesting to look at the population numbers on NGC and PCGS, seeing what percentage of new coins failed to grade MS70, the newer Morgan and Peace dollars for example. I've sent several of them back to the mint for refund due to scratches, pits.
