This error looks like a pierced nipple. The mint mark is looped into the Cud (anomaly) anchored?

Looks like D over zinc rot - not an error.

It's zinc rot, not an error. The zinc core got exposed by a split plating issue. Just PMD. https://www.error-ref.com/split-plating/
