How does something like this possibly happen and how is it legal to own? I would think this would have to be done from someone inside them in and smuggled out. I didn't think it was possible for a planchet bigger than a cent to be inserted for striking without destroying it.

I've groused about these fabrications for years. TPGs should use a special label if they stoop to slabbing them for they are most certainly not "errors".

It has to be an intentionally made so called "error" coin. Mint assisted "error". The chances of that happening truly by accident is super slim.
