The Greek on the reverse reads "PAVTAL", so it's most likely to be from the city of Pautalia, in Thrace (the modern-day town of Kyustendil, Bulgaria).

The city was known for it's hot springs and it's wine; the unknown object may have had something to do with either industry. I
think it's a snake coiled up sitting on top of an altar, similar to
this one, listed under Caracalla about halfway down the page I linked to above, but your emperor is bearded. I can't read the emperor's name (all I can read is his titles AV KAI), but it's probably Septimus Severus, possibly Commodus or an earlier emperor.
